TRANSCRIPT REQUESTS

Effective December 2008, we are now offering a new service to make ordering transcripts easier for you.  It’s also more efficient for your school, and will deliver your transcripts to the colleges you choose in the format they prefer.  Your PSAE/ACT and your last National ACT / SAT scores will be included, if you graduated after 2000. 

CURRENT STUDENTS:  You do not pay for the transcripts online.  Bring the Maine West Transcript Request Form, Counselor Forms, Waivers &/or Fee ($3.00 per request) directly to the Registrar’s Office.

FORMER STUDENTS:  If you graduated after 1990, you may go directly online to register. Your transcripts will be sent directly through DOCUFIDE for a $5.00 fee paid online.  (If you graduated before 1990, please see the information below for sending your request by mail.)

To send your transcripts electronically:

1.    Go to the TRANSCRIPT REQUEST box directly on the left-hand side of our Maine West website.

2.    “Sign Up” to register and begin your order, or “Sign In” if you have used DOCUFIDE before.

3.    Click on the circle for either currently enrolled, or not currently enrolled students

4.    Type in your year of graduation

5.    Type in your student ID or Social Security number (SSN)

6.     Click on “Save & Continue”

7.     Follow the instructions for adding the required information

8.     Enter your e-mail address and create a password

9.     Click “Continue”

10. Reply to the e-mail message from DOCUFIDE

11.  Follow the instruction to add colleges or scholarships to                         

       your list of transcript requests. 

12.  Confirm your order and print a copy for your records.

If you need to mail a request, you can print our request form or mail a note as a written release.  In the release, please include:

Student’s name when they attended school

Married name (if that applies)

Year of graduation

Social security number

Where to mail the transcript

Student’s SIGNATURE

*    $5 cash/check per transcript *

Please note we do not accept credit cards, e-mail or faxed releases.  If the transcript is for college, it should be mailed directly to the college.  Very few colleges, scholarships, unions and jobs allow you to hand-carry transcripts.  If it is to be mailed to your home, be sure to leave the transcript in the sealed envelope, to be considered “official”.  Be sure to allow enough time to meet your deadlines.

If a college needs “official” test scores from the testing agency, or you graduated before 1999, you will need to follow-up by contacting ACT, SAT or AP direct.  You can contact the testing agencies by phone or on their websites.
